PMI adoption index: How are the measurements calculated?

The table below explains the calculations used for each of the measurements that make up the PMI adoption index.
The Factor value indicates the measurements weighting, or impact, on total score. A measurement with a higher factor will have a greater impact on a department’s index score. The highest possible factor is 35, and the lowest is 3.
